"someone has committed suicide on the rail lines" -- not necessarily; I think they use it for any time a person ends up on the track which (a) might not be fatal; (b) might not be deliberate (drunken stumbling; pushed). I'm not entirely sure the person has to end up under a train.

I think there's something missing from your answer: I get that lids prevent spillages, but why would mismatched lids help? A passenger cup with a fitting lid would suffice, so why use different ones for the pilots?

Hmmm. Good question. Unknown. This has happened sufficiently often (in my experience) with budget airlines that they can't all be inop jet bridges.
